What does 'tier execution' mean versus just advising on what to do?

We own the requirements, not just the list. That means assigning each open item to a named person, producing the documentation AWS will accept, coordinating the technical validations, and holding a weekly rhythm until the tier changes. What are the main reasons firms fail to advance despite meeting most requirements?
